Talk to Your Hospital Social Worker
Ask your hospital case manager, social worker, or patient navigator to contact us on your behalf — or fill out the application below with their contact information included. Their verification is required to confirm your loved one's admitted status.
Submit Your Application
Complete the secure application form below. You'll be asked to provide the patient's name, hospital and unit, the name and contact information of the attending social worker or case manager, and a description of your family's needs.
We Review & Respond
Our team reviews requests within 24–48 hours and works to deploy resources as quickly as possible. You will hear from us directly once your request has been reviewed.
Who We Serve

To ensure fairness and protect families we serve, all assistance is distributed via direct vendor payments or restricted vouchers — we do not provide direct cash assistance. Lodging is paid directly to the hotel or hospitality house. Gas and parking assistance is issued through vouchers. This protects both applicants and our organization.
